Microsoft recently announced the finalists and winners of its 2021 Partner of the Year Awards. We’re delighted to share that Perficient was recognized as a finalist for the Healthcare Partner of the Year Award. 2021 Microsoft Partner of the Year Finalist_Healthcare Award

This honor positions Perficient among a global field of top Microsoft partners that have demonstrated excellence in innovation and implementation of customer solutions based on Microsoft technology.

Awards were classified in several categories, with honorees chosen from a set of more than 4,400 submissions from more than 100 countries. Recognition for Reimagined End of Life and Palliative Care

As the world grappled with the onset of COVID-19, healthcare organizations faced challenges to treat patients with the life-threatening virus while finding new ways to serve patients in need of preventative care or treatment. When the largest single-source provider of end-of-life care in the U.S. needed to make virtual patient care a reality, Perficient was the partner of choice.

Working with this specialized healthcare provider presented some unique challenges – especially during a pandemic. Fortunately, our broad and deep healthcare expertise served as a solid foundation for understanding the needs of patients as well as our client’s business.

Our team learned that approximately 80 percent of the provider’s patients want to receive care in their homes. Interdisciplinary teams comprised of nurses, social workers, chaplains, and clinicians deliver care for terminally ill patients, which is a delicate balance to provide safe interactions while also satisfying patients’ wishes.

With an understanding of our client’s challenges, we built a tailored solution on Microsoft Teams and Power Apps. The solution enables patient care teams to collaborate and share real-time information about patients’ needs wherever they are – including their homes. Within a single interface, the virtual care platform combines team meetings and patient consults, pushes updates and alerts from patient visits, and streamlines appointment scheduling.

The healthcare provider now has a scalable solution that improves continuity of care by avoiding after-hours intervention, restructures workflows for patient care teams, and reduces care costs.
