Perficient has created a patient portal to showcase how Oracle WebCenter and Fusion Middleware technology along with mobile applications can involve the patient in their own care to proactively monitor test results such as blood pressure, blood glucose, etc. through a data journal application.  Setting the expectation that vitals are monitored is nothing new, but automating the log and providing workflow to remind the patient and / or alert the care taker when test results are missing or out of normal ranges can be the difference between a trip to the doctor and a trip to the emergency room!
