Escalating costs and suboptimal quality of care are indicators that the healthcare system is broken. Preventative care is being pushed aside and traded for costly ER visits and inpatient hospital stays. The problem does not end here, however; once patients receive the necessary inpatient care, many are discharged to only end up back in the hospital -a costly and unsustainable situation.
This is a detrimental cycle that has gained momentum and come to the forefront of healthcare issues. To overcome this problem, how care is rendered is changing. This exploratory paper examines the role of PHM, the rise of PHM, necessary technology features, and the future of PHM as a solution to our ailing population and tight pocket books.
Research suggests that an effective healthcare model focus solely on evidence-based medicine. Similarly, many believe that preventative care does not impact cost and quality as well as programs that merge evidence-based medicine with a healthcare program. Organizations that are focused on containing costs and increasing quality are turning to PHM to meet their objectives.
